Care Manager RN Per Diem Day (Hiring Immediately)

Care Manager RN at Providence Queen of the Valley Medical Center in Napa, CA. This position is Per Diem and will work 8-hour, Day shifts.

The RN Case Manager is responsible for carrying out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care. The RN Case Manager assists patients in the utilization of appropriate health care services. Position may require Utilization Management and Discharge Planning, Chronic Disease Case Management based on inpatient settings.

Required Qualifications:

• Coursework/Training Recent completion of a post-graduate level curriculum focusing on case management. Or equivalent
• Current clinical experience with 2 years clinical experience in an acute care hospital setting, or
• 1 year current experience in an inpatient or outpatient case management setting.
• Upon hire: California Registered Nurse License
• 2 years Case management, discharge planning, or utilization management experience in the acute, sub-acute, home health setting or managed care environment.

Preferred Qualifications:

• Bachelor's Degree Nursing. Required to obtain within 3 years of hire date
• 2 years Case management, discharge planning, or utilization management experience in the acute, sub-acute, home health setting or managed care environment.
• 3 years Acute Care Experience

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
