Family Nurse Practitioner

Location: Mishawaka, IN 46544

Position Overview

Family Nurse Practitioner (FNP) to provide evidence-based primary and family healthcare in a patient-facing clinic setting.

The Nurse Practitioner will work collaboratively with a dedicated clinical care team to provide comprehensive care to a panel of patients. Responsibilities include patient assessment, diagnosis, treatment, chronic disease management, health counseling, and disposition planning.

IMPORTANT: Candidates must have at least 1 year of recent Primary Care or Urgent Care experience as a Nurse Practitioner. Candidates without this experience should not be submitted.

Schedule

Clinic Hours:

• Monday–Friday: 8:00 AM–7:00 PM
• • 1-hour lunch
• Saturday: 9:00 AM–5:30 PM
• • 30-minute lunch
• Sunday: 9:00 AM–4:30 PM
• • 30-minute lunch

Expected Schedule:

• Full-time
• Approximately 30–35 hours per week
• Every other weekend
• May require weekend and holiday rotations.

Key Responsibilities

• Provide primary and family evidence-based care, including:
• • Patient assessment
  • Diagnosis
  • Treatment
  • Management of health conditions
  • Health counseling
  • Disposition planning
• Perform routine examinations and evaluations.
• Diagnose and treat injuries and illnesses.
• Provide chronic disease management and preventive healthcare.
• Counsel patients regarding healthy lifestyle choices and wellness.
• Perform diagnostic testing as appropriate.
• Coordinate care with other healthcare professionals to establish comprehensive treatment plans.
• Encourage patients to actively participate in their healthcare and overall well-being.
• Provide services including health screenings, behavioral health and wellness support, and chronic condition management.
• Manage patient appointments and follow-up.
• Verify insurance and collect payments when required.
• Maintain accurate patient documentation.
• Assist with clinic opening and closing procedures.
• Maintain inventory and a clean, safe clinical environment.
• Work autonomously while collaborating with the broader clinical care team.
• Support quality measures related to chronic disease management.

Required Qualifications

• Minimum 1 year of recent Primary Care experience as a Nurse Practitioner in:
• • Family Medicine
  • Primary Care
  • Urgent Care
• Master's degree from a Family Nurse Practitioner program .
• Current national board certification.
• Current NP license in the state of employment.
• NP license must be in good standing.
• Strong clinical assessment, diagnosis, and treatment skills.
• Ability to practice autonomously.
• Strong verbal, written, and electronic communication skills.
• Strong organizational and multitasking abilities.
• Self-motivated with strong initiative and problem-solving skills.
• Ability to work effectively within a clinical care team.
• Proficiency with healthcare information management and technology.
• Willingness to obtain additional/multi-state licensure when required.
• Verifiable High School Diploma or GED required.

License & Certification Requirements

• Current Indiana NP/APRN license required for this position.
• Current national board certification required.
• BLS certification required.
• BLS must be issued by American Heart Association (AHA) or American Red Cross (ARC).
• DEA registration may be required during onboarding.
• License and BLS must be submitted with the candidate's resume.
