Outpatient Clinic Nurse (RN) - PRN Rehabilitation Hospital of Indiana (RHI) | Outpatient Rehabilitation | Full‑time | On‑site About the role If you are a Registered Nurse who enjoys combining clinical expertise, patient education, and interdisciplinary collaboration, this is an opportunity to play a meaningful role in specialized outpatient rehabilitation care. At the Rehabilitation Hospital of Indiana (RHI), the Outpatient Clinic Nurse supports patients across a variety of outpatient rehabilitation programs and specialty clinics. This role blends hands‑on clinical care with patient education, procedural support, care coordination, and collaboration with physicians and therapists to help patients achieve the best possible outcomes. You’ll work closely with an interdisciplinary team while supporting patients through evaluations, procedures, follow‑up care, and ongoing rehabilitation needs in a nationally recognized rehabilitation hospital environment. What you’ll do Perform nursing assessments for outpatient rehabilitation patients, integrating acute rehabilitation and age‑related needs while completing supporting clinical documentation. Provide patient and family education according to program protocols, ensuring patients understand treatment plans, procedures, and discharge instructions. Assist with outpatient specialty clinics, evaluations, and procedures while collaborating closely with physicians and therapists regarding patient care and re‑evaluation needs. Collect patient data for screenings and assessments, monitor patient response during procedures, and support pre/post‑procedure care and education. Monitor patient vital signs, clinical status, and responses during special interventions and outpatient procedures. Communicate with referring physicians regarding medical history, patient concerns, and continuity of care needs. Support coordination of care across providers, therapists, nursing staff, and outpatient rehabilitation services. Participate in quality improvement initiatives, competency activities, and program evaluation efforts to support Joint Commission, CARF, CMS, and organizational standards. Maintain clinical competency and evidence‑based nursing practice within the outpatient rehabilitation setting. Support a culture focused on patient safety, collaboration, professionalism, and exceptional patient experience. Qualifications Required Active Indiana Registered Nurse (RN) license. Graduate of an accredited School of Nursing. Current CPR certification in accordance with American Heart Association standards. Recent clinical nursing experience in acute care, outpatient services, rehabilitation, medical‑surgical, procedural, or related settings. Strong communication, patient education, critical thinking, and clinical assessment skills. Proficiency with computer systems and Microsoft 365. Preferred Bachelor of Science in Nursing (BSN). Experience supporting outpatient clinics, procedural areas, rehabilitation services, or physician practice settings. Experience with moderate sedation, special procedures, or interventional care environments. Familiarity with rehabilitation patient populations and interdisciplinary care models.
