The Role The Home Health Aide (HHA) provides personal care and supportive services to patients in their homes under the supervision of a Registered Nurse (RN) or licensed therapist, in accordance with the patient's individualized Plan of Care. The HHA is responsible for promoting patient comfort, safety, independence, and dignity while adhering to all Agency for Health Care Administration (AHCA), state, and federal regulations. Essential Duties & Responsibilities Provide personal care, including bathing, grooming, dressing, oral hygiene, toileting, and skin care. Assist patients with ambulation, transfers, positioning, and range-of-motion exercises as directed. Assist with meal planning, preparation, feeding, and monitoring nutritional intake. Perform light housekeeping related to the patient's care, including laundry, changing bed linens, and maintaining a clean, safe environment. Remind patients to take medications as directed (without administering medications unless permitted by law and agency policy). Observe and promptly report changes in the patient's physical, emotional, or mental condition to the supervising clinician. Accurately document all care provided, patient observations, and completed visits in accordance with agency policies and regulatory requirements. Follow the patient's Plan of Care and accept assignments only within the scope of practice. Maintain patient confidentiality and comply with all HIPAA requirements. Use proper infection prevention and control practices, including hand hygiene and appropriate use of personal protective equipment (PPE). Report any patient safety concerns, abuse, neglect, or unsafe living conditions immediately. Maintain effective communication with patients, families, supervisors, and other members of the healthcare team. Attend mandatory in-services, competency evaluations, and continuing education as required. Qualifications Current Florida Home Health Aide Certificate (minimum 75-hour training program) or current Florida Certified Nursing Assistant (CNA) license. Current CPR certification. Current Level 2 background screening meeting AHCA requirements. Valid driver's license, reliable transportation, and current auto insurance. Ability to travel throughout the assigned service area. Ability to read, write, and communicate effectively in English (bilingual preferred but not required). Ability to lift, transfer, and assist patients with mobility up to 50 pounds with or without reasonable accommodation. Knowledge, Skills & Abilities Knowledge of basic personal care techniques. Ability to recognize and report changes in patient condition. Excellent interpersonal and communication skills. Compassionate, dependable, patient-centered approach to care. Strong organizational and time-management skills. Ability to work independently while following clinical direction. Physical Requirements Frequent standing, walking, bending, kneeling, reaching, and lifting. Ability to assist patients with transfers and mobility. Visual and hearing ability sufficient to assess patient needs and maintain safety.
