Job Summary:

The Physical Therapist provides patient-centered rehabilitative care to improve mobility and function. They evaluate, develop, and implement treatment plans tailored to individual needs in a skilled nursing facility setting.

Responsibilities:

• 
  Assess patients' physical conditions and develop personalized treatment plans.
  
  
• 
  Implement therapeutic interventions to promote mobility, strength, and range of motion.
  
  
• 
  Monitor patients' progress and adjust treatments accordingly.
  
  
• 
  Educate patients and caregivers on exercises and care strategies.
  
  
• 
  Document patient progress and maintain accurate clinical records.
  
  

Qualifications:

• 
  Hold an active state license as a Physical Therapist.
  
  
• 
  Bachelor's or Masters degree in Physical Therapy from an accredited program.
  
  
• 
  Proven experience working with SNF residents or in a similar rehab setting.
  
  
• 
  Strong assessment and clinical reasoning skills.
  
  
• 
  Ability to communicate effectively with patients, families, and team members.
