Responsibilities Conduct Start of Care comprehensive assessments and complete OASIS timely Draft care plans, reconcile patient medications, and ensure orders for care are accurate and complete Prepare accurate, orderly, and timely documentation Coordinate care with patient and provide proper reports to Case Manager Provide health care instructions to the patient/family as appropriate per assessment and plan Qualifications About You: An active RN license in the state in which practicing or New Grad with eligibility to take NCLEX.
