Position Summary We are seeking a Physical Therapist to provide in-home care for Medicare Part B patients. Help patients regain mobility, manage pain, and live independently—all from the comfort of their homes. Key Responsibilities Assess patients in their homes and create personalized treatment plans. Provide skilled therapy to improve mobility, strength, balance, and independence. Educate patients and caregivers on exercises, safety, and fall prevention. Document progress and ensure compliance with Medicare Part B regulations. Collaborate with the home health / therapy care team to coordinate care. Adjust treatments based on patient progress and needs. Qualifications Active Physical Therapy (PT) license in the state of practice. Experience in home health or outpatient therapy preferred. Knowledge of Medicare Part B documentation and regulations. Strong assessment, treatment planning, and communication skills. Reliable transportation and valid driver's license. Comfortable working independently and managing a flexible schedule. This position requires Level 2 background screening through Florida's Care Provider Background Screening Clearinghouse.
