divh2Registered Nurse/h2pWe are seeking a dedicated Registered Nurse to join our Home Health team, to assume responsibility and accountability for the application of the nursing process and the delivery of patient care. The Registered Nurse (RN) demonstrates the ability to make clinical judgments in an effective and efficient manner under the direction of the Director of Clinical Services./ppPlease note: We are specifically seeking a candidate who is available to provide 24-hour or long time shifts nursing care coverage as needed. Flexibility and availability for day and night shifts are essential in this case./ph3Responsibilities/h3ulliUtilizes the nursing process to assess, plan, implement and evaluate patient care./liliAssesses signs and symptoms indicating physiologic and psychosocial changes in the patients condition./liliCollects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses./liliDocuments the patients plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions./liliPerforms interventions according to identified priorities, plan of care, and the hospital policies and patient care outcome standard./liliRevises the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions./liliUses clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor including establishing priorities./liliOther Registered Nurse (RN) duties as assigned./li/ulh3Requirements/h3ulliCurrent Registered Nurse (RN) License for the state in which the nurse practices./liliCurrent Health Certificate (per facility Registered Nurse (RN) contract or state regulation)./liliCurrent PPD or Chest X-Ray./liliCurrent BLS card./liliOne year prior Registered Nurse (RN) experience preferred./liliAvailability for 24-hour care coverage is required.
