Growing together.In this Field Based HSS Coordinator role will be an essential element of an Integrated Care Model by relaying the pertinent information about the member's needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs. This is a Field Based position with a Home-Based office.If you reside within 100 miles from Hidalgo County, TX and surrounding areas you will have the flexibility to work on an office-based/Field schedule as you take on some tough challenges.Primary Responsibilities:Complete face-to-face assessments in members' homes to address unmet needsTravel up to 75% of the time, including travel up to 100 miles from the assigned work location, to meet business needsMeet documentation and turnaround times based on state contractual requirementsTime management and organizational skillsCoordinating members' servicesAbility to create, edit, save and send documents, spreadsheets and emailsReside within commutable distance of assigned dutiesWorks on Service PlanYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear directions on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Must possess one of the following: High School Diploma/GED AND 5+ years of experience working with ABD/SSI population within a clinical health care roleAssociate's degree with 3+ years of clinical experienceUnrestricted LPN/LVN license in the state of TX with 3+ years of clinical experienceCHW certification with 3+ years of clinical experienceIntermediate level of proficiency with Microsoft Office, including Word, Excel, and OutlookHigh speed internet in place of residenceAccess to reliable transportation and valid US driver's license with the ability to travel up to 75% of the time in this 'assigned area' to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, nursing facilities or providers' officesPreferred Qualifications:LVN/LPN License in state of TexasAbility to create, edit, save and send documents, spreadsheets and emailsReside within commutable distance of assigned dutiesWorks on Service PlanPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $24.00 to $43.00 per hour based on full-time employment.
