Physical Therapist (PT) – Skilled Nursing Facility / Assisted Living Position Type: Full-Time Compensation: $39.00–$58.00/hour (DOE) New Graduates Encouraged to Apply We are currently seeking a full-time Physical Therapist (PT) to join a skilled nursing and assisted living rehabilitation team. Key Responsibilities Perform comprehensive evaluations of residents to assess strength, mobility, balance, coordination, and functional ability. Develop and implement individualized physical therapy treatment plans based on clinical findings and physician orders. Provide evidence-based therapeutic interventions including therapeutic exercise, gait training, manual therapy, pain management techniques, and modality use. Document evaluations, treatments, and resident progress in compliance with regulatory, facility, and professional standards. Collaborate with nurses, physicians, occupational therapists, speech-language pathologists, and other healthcare team members to ensure coordinated care. Communicate effectively with residents and families regarding treatment plans, progress, and goals of care. Ensure compliance with all federal, state, and facility policies, including HIPAA, Medicare/Medicaid documentation guidelines, and infection control protocols. Participate in interdisciplinary care planning meetings and facility in‑services as needed. Qualifications Required Graduate of an accredited Physical Therapy program (Bachelor’s Degree required; Master’s Degree preferred). Current or eligible state Physical Therapy license. Strong communication, clinical reasoning, and documentation skills.
