This flexible role allows you to conduct essential home health risk assessments for Medicare Advantage members in Quincy, Payson, Camp Point, and nearby locations. Your professional expertise will directly impact the health and well-being of community members all while you maintain full control over your work-life balance.

Schedule:

• Flexible shifts available Monday through Sunday
• Opportunity to design your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive home health risk assessments in patient homes, typically 45 minutes per visit
• Assess medical history, current health status, and medication usage
• Educate patients about their conditions and medications
• Identify care gaps and provide actionable recommendations to enhance patient outcomes
• No responsibility for prescribing medications or developing treatment plans

Qualifications:

• Active Nurse Practitioner license in the state of Illinois
• Board certification from ANCC or AANP
• Current CPR (BLS) certification required
• At least 6 months of recent nurse practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited program

Youll be compensated $80-$90 per completed evaluation, with additional mileage reimbursement and payment for administrative time.
