Registered Nurse Non-Certified Case Manager – Case Management

Schedule: Day Shift: 7:30 AM - 4:30 PM, Weekend Rotations

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40

Requirements:

• Montana Licensure (Registered Nurse)
• American Heart Association BLS
• Minimum of 2 years of nursing experience
• Utilization review experience

Roles & Responsibilities:

• Collaborate with physicians and multidisciplinary team members to develop and implement care plans for patients.
• Monitor patient progress through assessments and reviews of patient records.
• Identify and resolve barriers to effective patient care.
• Actively participate in discharge planning.
• Maintain an ongoing caseload for regular patient outreach and management.
• Promote integration of services for members, including behavioral health care and community support.

Compliance / Notes:

• Regular attendance is required.
• Position may involve working beyond a standard 40-hour workweek, including weekends and on-call shifts.
• Physical requirements include the ability to lift up to 50 pounds and maintain various postures throughout the workday.
• Exposure to bloodborne pathogens, chemicals, and infectious diseases may occur.
