Case Management Registered Nurse (RN)

We are currently seeking a skilled Case Management Registered Nurse to join our dedicated team. This position is located in Pasadena, TX, and involves working within the Case Management and Social Work Department to ensure the effective management of patient care across the entire facility.

Unit Information

• Department: Case Management and Social Work
• Number of Staff: 22 personnel
• Patient Ratios: 20-25 patients per RN, 50 patients per Social Worker
• Type of Equipment: Standard office equipment including computers and phones
• Electronic Medical Records (EMR): Meditech/Meditech Expanse starting 8/13

Hiring Profile

• Required Experience: Minimum of 2 years of hospital Case Management experience
• Preferred Experience: Additional 2 years of hospital Case Management experience is highly valued
• Types of Duties:
  • Discharge planning
  • Assessment using Interqual criteria to confirm patient status
• Personality Fit: Candidates should be flexible, proactive, and comfortable asking questions.
• Required Certifications: Must possess a current RN license.
• Floating Areas: Potential to float to any med-surge or ICU unit as needed.

Work Environment

• No weekend requirements.
• Dress Code: Business casual attire is required; scrubs are not permitted. A plain white lab coat may be worn if owned.
