NOW HIRING: Home Health RN Case Manager

Location: Bowling Green, KY

Coverage Area: Up to 50 miles

Hours: Monday - Friday, 8:00 AM - 5:00 PM

Pay: $53.00 per hour (Not Per Visit)

Scheduled Pay: Weekly

Mileage Reimbursement: $0.43 per mile

Counties covered: Warren, Logan and Simpson

Benefits: Health, Dental, Vision, Life Insurance, and Short-Term Disability after 90 days

Home Health RN Case Manager Responsibilities

• Productivity Expectation: Approximately 30 points/units per week
• Conduct comprehensive patient assessments in the home setting and develop individualized care plans based on patient needs and physician orders.
• Perform skilled nursing services including medication administration, wound care, injections, disease management, and ongoing patient monitoring.
• Monitor patient conditions, identify changes in health status, and communicate updates to physicians and interdisciplinary care team members.
• Educate patients and caregivers regarding diagnoses, medications, treatment plans, disease prevention, and health maintenance strategies.
• Coordinate care with physicians, therapists, social workers, caregivers, and other healthcare professionals to ensure continuity of care.
• Manage medication orders, reviews, monitoring, discontinuation, and documentation in accordance with physician orders and agency policies.
• Complete accurate and timely clinical documentation within 24 hours of patient visits.
• Weekend on-call rotation required.

Home Health RN Case Manager Requirements

• Active, unrestricted Kentucky Registered Nurse License required
• Valid driver's license and reliable transportation required
• Minimum 2 years of Home Health experience required
• Minimum 2 years of OASIS experience required
• Minimum 2 years of Homecare Homebase (HCHB) experience required
