Use professional nursing knowledge, critical thinking skills and dialysis experience to serve as a resource to staff, Physicians and Physician's Assistants.Minimum Qualifications:Education and Experience:Graduate of an accredited nursing program required; Associates Degree in Nursing (ADN), Bachelor of Science in Nurse (BSN) and/or Master of Nursing (MSN)Current state licensure as a Registered NurseCurrent CPR certification (AHA BLS)18 months of clinical nursing experience to patients on maintenance dialysis within the last 24 months.Required Skills And Competencies:To maintain on-call status on an intermittent or regularly scheduled basis.Ability to satisfactorily communicate in English including reading, writing, speaking, and understandingComputer proficiencyEssential Duties And Responsibilities:In conjunction with the Vice President, ensures dialysis unit operates in compliance with all ESRD, HCFA, federal, AAMI, OSHA and company policies and proceduresResponsible for training and development of Skilled Nursing Facility staff while providing clinical oversight to non-licensed support team delegating tasks as appropriateKnows and follows policies and procedures under CMS ESRD and state regulations and Rendevor Dialysis, monitors staff compliance of sameMonitors and assures patients receive adequate medical attention during dialysis treatments and pertaining to dialysis care while a patient of the Skilled Nursing Facility in the dialysis unitConducts comprehensive assessments that include the medical, behavioral, pharmaceutical, and social needs of the patient, identify gaps in care and barriers to good healthBased on this assessment, and in conjunction with the patient care team, create and implement a care plan that will address identified needs, remove barriers to care, and improve the health of the patientCoordinates care by serving as the advocate and resource for the patient, their family, and their provider(s)Facilitates continuity of care, spanning settings such as the home, hospital, skilled nursing facility, and acute care facilityManages patients during periods of transitions of care minimize avoidable readmissionsAssesses the patient's knowledge of their renal condition and provide education and self-management supportProvides ongoing reassessment and follow-up to improve patient outcomesReviews and maintains updated policies and procedures on a regular basisDevelops and monitors quality control measures to assure continuous quality of patient careFacilitates Quality Assurance MeetingsParticipates in the IDT and QAPI committee and attend meetings as needed and/or requestedIdentifies and reports concerns, issues, and opportunities for improvement when identifiedParticipates in performance improvement projects within licensure, certification, and scope of practiceParticipates in annual QAPI training and other continuing education, as assigned, and requiredProvides suggestions for alternate strategies for indicators not improvingAssists in building and maintaining a facility/clinic culture of safety and qualityReports errors and near misses so that steps can be taken to prevent recurrenceReports any negative feedback, complaints, grievances from other staff members, patients, and familiesUses evidence-based guidelines and proper infection control practicesMaintains open communication with Skilled Nursing Facility Administrator, Medical Director and Rendevor Dialysis management and staffUtilizes resources to improve skills, knowledge, and practice to assure continuous quality of patient carePromotes a teamwork approach by offering information, advice, and assistance to all unit staff members in a positive, courteous, and cooperative mannerChecks email at least weekly and responds appropriately and timely; uses company-provided email for all work-related email communicationsMaintains regular and predictable attendance and punctualityAdheres to all company policies and proceduresPerforms other duties as assignedSupervision:This position will report directly to the Unit Leader.Work Environment:This job exists in the medical unit of a post-acute facility. This role may include exposure to chemicals and blood-borne pathogens. This role routinely involves the use of both medical and standard office equipment.Physical Demands:The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to handle or feel; and reach with hands and arms. This position requires the ability to push, pull or lift supplies and products up to 50 pounds. Specific visual abilities required by this job include close vision and ability to adjust focus.
