Covenant House Incorporated, a community-based healthcare organization dedicated to providing high-quality, patient-centered care, is seeking a full-time Certified Community Health Worker (CCHW) to join our team.

The Certified Community Health Worker will be a vital and trusted member of the healthcare team, serving as a liaison between patients, the community, health, and social service providers. The successful candidate will help individuals navigate the healthcare system, connect with community resources, and promote improved health outcomes through education, advocacy, and care coordination.

Core Responsibilities:

·         Building trusting relationships with patients and families.

·         Conduct outreach by phone, in the community, and during home visits as appropriate

·         Assist patients in navigating healthcare and social service systems

·         Re-engage inactive patients into healthcare regimens and promote preventive care participation

·         Complete patient intake screenings and assessments

·         Identify barriers affecting health outcomes

·         Support the development of patient-centered healthcare action plans

·         Coordinate services with healthcare providers and interdisciplinary care teams

·         Connect patients with daily living resources

·         Participate in quality improvement and program evaluation activities

·         Demonstrated experience working with diverse com

·         Excellent communication and time management abilities

·         Computer literate

·         Ability to travel throughout Philadelphia and the surrounding communities

Education/Experience Requirements:

·         High School Diploma/Equivalent, associate’s degree in public health, Human Services, Healthcare, or related field preferred

·         Community Health Worker Certification preferred

·         Experience in healthcare, case management, outreach, care coordination, social services, or community-based programs

·         Knowledge of Philadelphia community resources and social service systems

·         Bilingual in English and Spanish is a plus
