What You'll Do
• Provide skilled nursing visits in accordance with physician orders and established care plans
• Assess patient needs and document clinical findings accurately and thoroughly
• Maintain complete and compliant medical records
• Educate patients and families regarding treatment plans and ongoing care needs
• Communicate effectively with physicians, caregivers, and interdisciplinary team members
• Monitor patient progress and report changes in condition as appropriate

Qualifications
• Graduate of an accredited Practical Nursing or Vocational Nursing program
• Current Texas LVN license in good standing
• Minimum of one year of clinical nursing experience
• Minimum of one year of home health experience

Pay Details: $70,000.00 to $87,000.00 per year
