Job Summary:

Woburn Rehabilitation and Nursing Center, a dynamic and innovative healthcare facility, is seeking a dedicated and compassionate Registered Nurse (RN) to join our team of caring professionals. As an RN, you will provide high-quality, resident-centered care while collaborating with an interdisciplinary team to develop individualized care plans and promote positive clinical outcomes. Responsibilities:

• Provide direct nursing care to residents, including administering medications, obtaining vital signs, and monitoring residents' conditions.
• Develop and implement individualized resident care plans in collaboration with the interdisciplinary team.
• Observe residents and promptly identify and report changes in condition.
• Communicate effectively with residents, families, physicians, and other healthcare team members to ensure coordinated care.
• Accurately document resident care, clinical observations, and interventions.
• Participate in quality improvement initiatives and support the facility's commitment to excellence in resident care.
• Follow facility policies, procedures, and applicable state and federal regulations.
• Promote resident dignity, safety, independence, and overall well-being.
• Demonstrate a commitment to the facility's mission, values, and standards of excellence.

Qualifications:

• Current, active, and unrestricted Massachusetts Registered Nurse (RN) license.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and interpersonal skills.
• Ability to collaborate effectively with residents, families, and interdisciplinary team members.
• Ability to manage multiple priorities in a fast-paced healthcare environment.
• Compassionate, dependable, and committed to providing high-quality resident-centered care.
• Previous skilled nursing, rehabilitation, or long-term care experience preferred but not required.
