Registered Nurse Case Management

Experience: Minimum 4 years of recent RN Case Management or Licensed Clinical Social Worker experience

License: New Jersey RN or multi-state RN or New Jersey SW license required

Certifications: BLS certification required through the American Heart Association

Must-Have: Minimum 4 years recent RN Case Management or Licensed Clinical Social Worker experience

Experience in Long-Term Acute Care (LTAC) or Short-Term Acute Care Client (STACH) setting

Experience with care planning, implementation, and discharge planning across acute care settings

Strong interdisciplinary team collaboration

Description: Coordinates and facilitates the care of the patient population through effective collaboration and communication with the Interdisciplinary Care Transitions team members. Follows patients throughout the continuum of care and ensures optimum utilization of resources, service delivery, and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment. Develops and implements care plans for individual patient needs. Evaluates patient needs and enhances the quality of patient management and satisfaction. Promotes continuity of care and cost-effectiveness through the integration of case management, utilization review and management, and discharge planning functions.
