Nurse Practitioner (NP) – Nursing Home Primary Care (Weekend Coverage)A medical group providing primary care services to nursing homes is seeking a Nurse Practitioner to provide weekend coverage for a skilled nursing facility located in the Canarsie section of Brooklyn.This role focuses on providing primary care to geriatric patients in a nursing home environment. The provider will manage patient visits, follow-ups, and coordinate care with facility staff to ensure continuity of care over the weekend.The provider will work weekends (Saturday and Sunday). Providers must be available between 8:00 AM and 6:00 PM, with flexibility in structuring their day based on patient volume and facility needs.• $500 – $600 per day• 1099 independent contractor• Additional earning potential depending on patient volumeProvide primary care services to geriatric nursing home residentsConduct routine and urgent patient visitsManage chronic conditions and acute medical issuesCoordinate care with nursing staff and facility leadershipParticipate in CCM and RPM programs when applicableDocument patient encounters in facility EMR systems (SigmaCare or PCC)Active New York Nurse Practitioner licenseActive Medicare and Medicaid provider IDExperience working with geriatric patients preferredSkilled Nursing Facility experience preferredRN background in a nursing home or long-term care setting is highly valued
