Remote Youth Care Coordinator Family-Centered Services

Harbor is seeking a Care Coordinator to support youth with complex needs and their families. The role is full-time, remote, and requires evening hours and weekend availability to engage families and attend community events. Qualifications include 3 years of relevant experience (or 2 years with an Associate or Bachelor degree; or 1 year with a Master degree) in community-based services, with competencies in family systems and case management.
