About the Position

Summary Long Term Care facility on site of hospital. Will be performing assessments, treatments, medication administration, notification to MD/NP of pertinent information, admitting/discharging, ADL assistance, Gastrostomy Tube care, Urinary Catheter care, Nebulized Treatment, Wound Vac care, CPAP care, Tracheostomy Care and other assigned duties.     Key Responsibilities

• Assess residents' physical, mental, and emotional health status.
• Develop, implement, and update individualized care plans.
• Administer medications, treatments, and injections as prescribed.
• Monitor residents for changes in condition and respond appropriately.
• Document assessments, treatments, and resident progress accurately.
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
• Supervise and support Licensed Practical Nurses (LPNs), nursing assistants, and other care staff.
• Educate residents and families regarding health conditions and care plans.
• Manage emergency situations and provide first aid or life-saving interventions when necessary.
• Ensure compliance with infection control, safety, and regulatory standards.

Requirements

Required

• Current Registered Nurse (RN) license in the applicable jurisdiction.
• Graduate of an accredited nursing program.
• Current CPR/BLS certification.
• Knowledge of geriatric nursing principles and chronic disease management.
• Strong clinical assessment and critical thinking skills.
