Physical Therapist (PT) – PRN

Agawam Campus is seeking a PRN Physical Therapist (PT) to join our rehabilitation team within a skilled nursing facility. This role offers flexible scheduling and the opportunity to work with residents focused on improving mobility, balance, strength, and overall functional independence.

Position Responsibilities:

• Complete physical therapy evaluations and provide individualized treatment sessions
• Address gait training, transfers, balance, strengthening, endurance, and mobility goals
• Collaborate with rehab staff, nursing, and physicians to support resident care plans
• Monitor resident progress and adjust treatment plans as needed
• Maintain timely and accurate documentation

Qualifications:
