Registered Nurse - Home Health

Job Description

Job Summary:
Provides comprehensive home health nursing care to patients based on physician orders, utilizing the nursing process to deliver high-quality care. Coordinates patient visits, supervises home health aides, and ensures compliance with agency policies.

Responsibilities:

• Assess patient needs and develop individualized care plans.
• Administer medications and treatments per physician directives.
• Supervise and evaluate home health aides and licensed practical nurses.
• Document patient progress and care activities accurately within required timeframes.
• Travel to patient homes within a 75-mile radius, maintaining timely schedules.
• Participate in team meetings and perform other duties related to patient care and documentation.

Qualifications:

• Valid RN license in the state of practice.
• Minimum of 1 year of recent adult home health or hospice experience.
• Proficiency with HomeCare HomeBase (HCHB) EMR system within the last 6 months.
• Strong clinical assessment and patient management skills.
• Ability to travel up to 75 miles from the clinic location.
• Reliable transportation, valid driver's license, and agency-required auto insurance.
