Case Manager-RN

Pay Range: $35.92 - $53.88 A successful candidate's actual pay rate will be based on several factors including relevant experience, skills, training, certifications and education. Job Description Summary Hospital Sisters Health System (HSHS) is seeking a Care Manager-RN to join our team. In collaboration with the patient/family, social workers, physicians, interdisciplinary team, and payer representative, the Care Manager-RN ensures patient progression through the continuum of care in an efficient and cost effective manner. Identifies, initiates and manages optimal care coordination and discharge planning to develop patient flow/throughput to enhance continuity of care, smooth and safe transitions, patient satisfaction, patient safety and length of stay management. Provides support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. Monitors the provision of cost effective care to ensure care is given in the most clinically appropriate setting. Facilitates compliance with regulatory requirements and organizational performance standards, including demonstration of core competencies. Position Specifics Department: Case Management Core Function: Nursing Schedule: Day shift, Full Time-40 hrs/wk, Mon-Fri 7:30-4:00pm, weekend rotation Facility: St. Elizabeth's Hospital Location: O'Fallon, IL Compensation that aligns with your experience Education & Experience Education: Associates degree or Diploma in Nursing is required. Bachelor's in Nursing is preferred. Experience: 3 years of clinical experience is required. Prior case management experience is preferred. Quality/Process Improvement experience is preferred. Registered Nurse (RN) license in the state of practice is required (IL or WI). Case Management certification (ACM CCM RN-BC) is preferred.
