Job Summary This position may qualify for a sign‑on bonus. Provide pharmaceutical care services to the patients including, but not limited to oversight, monitoring, preparation, dispensing and proper documentation of patient medication therapy, as well as providing drug information and overseeing dispensing systems. Additionally, ensure compliance with Federal, State, and local laws and regulations and may advise physicians on medication utilization. Responsibilities Review and monitor patient medication therapy and orders written for safety and efficacy, considering specific patient conditions and health status; advise the patient care team and document interventions. Verify orders and perform medication profile reviews, checking for duplications, drug‑drug and drug‑food interactions, allergies, drug appropriateness, dosing, and frequency. Prepare, label, and dispense medications for patients in accordance with physicians' prescriptions; ensure compliance with Federal, State, and local laws and regulations pertaining to dispensing. Check high‑risk IVs, renal dose‑adjusting medications, IV to PO conversions; initiate, modify, or discontinue therapy and mediate black‑box warning interventions. Ensure appropriate medications are available, properly procured, prepared, stored, and maintained. Supervise the preparation, maintenance, and dispensing process of drugs and supplies, including supervising technical personnel. Maintain relationships with medical and nursing staff; provide timely information on pharmaceutical supplies, drug usage, compatibility, state and federal regulations regarding drug controls, and Joint Commission standards. Provide collaborative practice management per facility‑approved protocols for anticoagulation, parenteral nutrition, vancomycin, and aminoglycoside consults. Assist with medication reconciliation and clarification of unsafe and unclear drug therapy orders; provide patient education on medication usage. Participate in the education and training of healthcare professionals (physicians, nurses, pharmacy technicians, and students) on medication use. Demonstrate regulatory compliance by timely completion of nursing unit inspections as needed. Maintain professional competence with the pharmacy department and state licensing requirements via staff development, independent study, and professional activities. Qualifications & Certifications Education: Pharm.D. or BS in Pharmacy. Experience: Preferred hospital experience. Certifications Required: Pharmacy license; AHA BLS, ACLS, PALS required if direct patient care responsibilities.
