Job Description

The Outpatient Physical Therapist evaluates all assigned patients, develops, implements, and modifies treatment plans in accordance with patient needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients. The therapist ensures the design of functional treatment programs that allow patients to effectively return to community activities and serves as an educator for patients, families, students, coworkers, and themselves.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Complete accurate and timely medical record documentation.
• Participate in the student program by providing supervision and evaluation.
• Develop and actively pursue a professional growth plan.
• Maintain professional licensure and adhere to the discipline-specific State Practice Act.
• Understand and engage with the organization's performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ years of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for physical therapy.
