Licensed Practical Nurse (LPN), Per Diem, Wayne County Nursing Home in Lyons, NY - Paterson, NJ

Location: Lyons, New York (Wayne County, 35 miles east of Rochester). On-site at a 192-bed county skilled nursing and rehabilitation facility.

Employment Type: W-2 hourly, per diem. You set your own availability.

Compensation: $38.00 to $42.00 per hour; nights and weekends paid at the top of the range. Time and a half on overtime and on all ten County holidays. Paid weekly.

Schedule: You choose. Eight paid hours per shift with a half-hour unpaid meal break (6:30 AM to 3:00 PM, 2:30 PM to 11:00 PM, 10:30 PM to 7:00 AM); twelve-hour shifts when available. Any day of the week, no minimum hours.

Agency Environment: County-operated skilled nursing and rehabilitation facility rated High Performing in Long-Term Care by U.S. News Best Nursing Homes 2025, with an RN on every shift.

Position Overview

$38.00 to $42.00 an hour, paid weekly, on shifts you choose, with up to $1,500 a year in tuition assistance toward your RN. That is the offer.

Wayne County's nursing home in Lyons is rated High Performing in Long-Term Care by U.S. News for 2025, and it needs LPNs who know a med pass and know their residents. You would run medications and treatments on your unit with an RN on every shift behind you, in a building where the leadership is on site and the residents know your name.

Compensation and Engagement Details

• $38.00 to $42.00 an hour. Experience sets your rate within the range, and every night and weekend shift is paid at the top of it.
• Time and a half. On overtime over 40 hours, and on all ten County holidays.
• Your time is protected. A confirmed shift the facility cancels two hours or less before it starts is paid for four hours. With more notice than that, the shift simply comes off your schedule.
• We pay you to stay and grow. Your New York license renewal and CPR renewal are reimbursed. Continuing education and certifications are supported. Tuition assistance of up to $1,500 a year toward your RN. Bring a nurse or aide onto the roster and there is a $500 referral bonus for you. All of it runs through CLFC University, our own training and development program, and the program terms come to you in writing before you accept.
• Scheduling that respects your life. Open shifts at the facility appear in the BRIDGE App on your phone. You tap the ones you want, you get a confirmation back, and that shift is yours. Nobody calls you at 5 AM to fill a hole, and nobody expects you to take a shift you did not choose. It also keeps your license and certifications on file and reminds you before anything expires.
• Paid weekly, and your orientation is paid too. Pay lands every week on County-approved time, with no payment holds. Orientation hours are paid at your rate. Fingerprinting, immunizations, screening and license verification are arranged and paid by CLFC, and paid sick leave accrues under New York law, and you are covered by CLFC's professional liability insurance on every shift. • A real person on the other end. Your Program Manager knows your name, answers the phone, and sorts out scheduling and pay questions without a ticket system.

Key Responsibilities

• Run the medication pass and deliver treatments per physician orders and facility policy.
• Observe, document and report changes in resident condition to the charge RN.
• Support and direct the CNAs on your unit within New York scope of practice.
• Keep shift documentation current in the facility's system.
• Follow the facility's infection control, safety and sign-in practice.

Required Qualifications

• Current, unrestricted New York State Licensed Practical Nurse license.
• Graduation from an approved practical nursing program and current BLS (CPR).
• One year of recent LPN experience.
• Able to complete the New York State Department of Health Criminal History Record Check and a drug screen, and current on the immunizations New York requires for nursing home personnel.

Strongly Preferred Qualifications

• Long term care or skilled nursing experience.
• IV certification.
• Availability for nights, weekends or short notice.
