Job Details: Job Title: Home Health RN Location: Buford, GA Setting: Home Health Pay Range: 2100-2300 Schedule: Monday-Friday 8-5 Contract Length: 13 weeks Candidate Type: Traveler Requirements: 1 years of Medicare certified Home Health experience as well as HomeCare HomeBase EMR and OASIS experience. Responsibilities: Provide skilled nursing care to patients in the home setting while supporting individualized care plans. Assess patient status, document clinical findings, and communicate updates to the care team as needed. Educate patients and caregivers on care routines, safety considerations, and ongoing health management. Coordinate care and help ensure timely follow-up with appropriate clinical and support resources.
