Develop and implement individualized treatment plans focused on mobility, ADLs, cognition, and engagement in meaningful daily activities. Deliver direct patient care, lead therapeutic sessions, and document progress in the chart system. Collaborate with an interdisciplinary team including nursing, PT, and other therapy disciplines to coordinate care. Reassess plans regularly, modify interventions as needed, and contribute to discharge planning and care transitions. Maintain compliance with regulations and facility policies while preserving resident confidentiality and dignity.
