Clinical Care Registered Nurse

M-F 8:00am - 4:30pm - on-call/holiday rotations

Requirements

• Bachelor's degree preferred in Science of Nursing; Associates degree required.
• RN–Nursing Diploma or Degree from accredited School of Nursing or University.
• Minimum of one year of experience working with the frail, elderly, or long-term care population.
• Minimum of one year home care experience preferred.
• Minimum of one-year wound care experience preferred.
• Current CPR certification by AHA or willingness to obtain.
• Must have reliable transportation, current valid driver's license, and car insurance.
• Must be fully vaccinated, or willing to be fully vaccinated against specified communicable diseases.

Responsibilities

• Provide direct nursing care to participants and manage complex clinical situations.
• Triaging emergent situations and applying advanced nursing knowledge.
• Monitor acute and chronic medical conditions in coordination with Primary Care Providers (PCPs).
• Utilize care plans to provide clinic and home-based direct care independently.
• Administer treatments, procedures, and medications as prescribed.
• Conduct regular home visits to provide care and treatment as ordered by the PCP, including wound care and lab draws.
