Amity Foundation

Amity Foundation invites compassionate individuals to join our ECM CHW team. The role focuses on care coordination, improving health outcomes, and reducing unnecessary healthcare utilization for students. You will bridge ECM students with medical and community resources, with field-based duties and local travel required. Required: degree in social work/psychology/public health or related field (or high school + 3 yrs outreach), plus 2+ yrs in client-facing work. #J-18808-Ljbffr
