Contract | Days | Monday–Friday, 8:30 AM–5:00 PM | One weekend per month required for visits | One major and one minor holiday required | No on-call rotation

Key Responsibilities

• Provide occupational therapy services to home health patients across the east Lake County coverage area
• Conduct an average of 4–6 patient visits per day
• Travel to patient homes within the assigned coverage area, with mileage reimbursement provided per IRS rate
• Participate in one weekend per month to support orthopedic cases and scheduled holiday coverage as required
• Adhere to the facility's internal call-off policy, including potential rescheduling of weekend or holiday shifts if a call-off occurs

Required Qualifications

• Active Florida Occupational Therapist (OT) license required
• Minimum of 1 year of occupational therapy experience
• Valid driver's license and current auto insurance required
• Must be able to provide at least 1 professional reference from entire work history
• SSN and date of birth required for credentialing

Preferred Experience / Skills

• Prior home health experience
• Experience with orthopedic patient populations
