RN Care Liaison — Home Infusion & Discharge Specialist

Soleo Health

Soleo Health in Atlanta and surrounding suburbs is seeking a Nurse Liaison to coordinate patient transitions from hospital to home or alternate site infusion therapy, working with discharge planners, case managers, physicians and internal teams to ensure safe, timely discharges. The role includes bedside education for patients and caregivers on IV therapy, device use, and care coordination, plus collaboration with hospital staff and referral sources to support adherence and smooth care #J-18808-Ljbffr Soleo Health
