Job Summary

Cuready Healthcare Staffing is seeking a qualified Registered Nurse - Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Conduct thorough patient assessments to guide care coordination.
• Develop and implement discharge plans to ensure continuity of care.
• Utilize Epic charting system for accurate documentation and care planning.
• Collaborate with healthcare teams, including social services and physical therapy.
• Maintain compliance with CMS, The Joint Commission, and core safety goals.
• Independently arrange DME, SNF, and transport as needed.

Qualifications

• Current CA RN license, or pending with licensure by start date.
• Minimum 2 years of acute care hospital experience in case management.
• Experience with HMOs, IPAs, and care coordination processes.
• Proficiency with utilization review and regulatory compliance.
• Ability to manage a caseload of up to 25 patients.
• Strong skills in discharge planning and multidisciplinary collaboration.
