Physical Therapist (PT) The Phoenix Rehabilitation and Nursing Center is part of the CareRite Centers Network and is looking for a talented and hard‑working Licensed Physical Therapist (Full‑Time) to join our ever‑growing rehabilitation team. The Phoenix is a facility with 400 beds and is proud of its state‑of‑the‑art Rehabilitation Gym servicing 60+ short‑term beds. The rehabilitation team consists of 30+ therapists spanning 20 years of experience with backgrounds in acute, sub‑acute, long‑term, and outpatient settings, providing an excellent setting for learning and growth. We are a 7‑day a week rehabilitation facility! In this role, you will have the opportunity to work with a diverse patient population on various sub‑acute care services including post‑surgical, orthopedic, neurology, post‑stroke, cardiac, and much more. Responsibilities Assess referred patients requiring rehabilitation services, including direct patient care to evaluate medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation. Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician. Set realistic and achievable goals for patients, document and record their condition, and educate patients and families in appropriate physical therapy methods. Qualifications / Requirements Graduated from an accredited physical therapy college or university. Hold a valid professional license and registration in the state of New York. Ability to supervise and oversee Physical Therapy Assistants. Experience with long‑term care and/or geriatric care preferred. Well versed in documentation writing, including evaluations, weekly progress notes, recertifications, and discharge documentation. Collaborates with all disciplines to provide patient‑centered treatment and care. Exceptional patient service skills and a demonstration of clinical excellence. Reliability, punctuality, adherence to scheduled hours, and professionalism at all times. Perform other duties assigned by the Director of Rehabilitation.
