In this role, you'll help deliver high-quality, member-centered care and play a key part in improving clinical outcomes, supporting care coordination, and enhancing the experience for both members and their caregivers. This opportunity is well suited to a compassionate medical assistant who is passionate about improving the health and well-being of older adults through personalized, holistic support.

Responsibilities

• Review community health worker appointment documentation for accuracy and completeness, escalating issues to the nurse case manager as needed.
• Support care coordination activities, including scheduling follow-up appointments and communicating with members or their caregivers.
• Mail and/or email patient care plans and educational materials as directed by case managers.
• Accurately update and maintain patient records within the electronic health record system.
• Protect patient confidentiality and support compliance with HIPAA and organizational policies.
