Assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's health needs throughout the continuum of care.Provides discharge planning through referrals and multidisciplinary planning and serves as a liaison between patient/family and the healthcare team to ensure necessary care is provided promptly and effectively. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks related to the coordination of care. A certain degree of creativity and latitude is required. Ensures that patients' unique needs are met in the most appropriate, timely, and cost-effective manner.The ideal candidate must be highly detail-oriented and goal-oriented, with superior analytic, critical-thinking, and time-management skills. They must have a positive, empathetic personality and the ability to work effectively with others as part of a multidisciplinary team in a complex care environment.
