Licensed Practical Nurse

Who We’re Looking For: Licensed Practical Nurse experienced in home health care, compassionate, dedicated, and eager to provide exceptional care in patients’ homes. Key Responsibilities Provide nursing care to patients in their homes, including administering medications, wound care, and other treatments. Monitor patients’ conditions, report changes to the care team, and assist in developing personalized care plans. Educate patients and families on health management, medications, and self‑care techniques. Collaborate with the healthcare team to ensure the highest quality care for each patient. Primary Job Duties Assisting with the assessment, planning, implementation and evaluation process according to the patient’s age and diagnosis. Effectively and efficiently assisting with patient care management in coordination with a multidisciplinary team. Appropriately communicating information in accordance with agency policies and procedures and discipline‑specific guidelines. Contributes to program effectiveness. Organizes and performs work effectively and efficiently. Maintains and adjusts schedule to enhance agency performance. Demonstrates a daily commitment to the values of the agency. Demonstrates positive interpersonal relations in dealing with all members of the agency. Maintains and promotes customer satisfaction. Effectively demonstrates the mission, vision, and values of the Agency on a daily basis. Job Specifications Education: Graduate of an accredited or approved school of practical nursing. Licensure / Certifications: Current license to practice practical nursing in the State of North Carolina; CPR certification required. Experience: Minimum one year practical nursing experience required; home health experience preferred. Essential Technical / Motor Skills: Hand/eye coordination for injections, computer use, patient transfer, and equipment handling; literate in English. Interpersonal Skills: Ability to develop positive interactions with patients, families, physicians, and staff. Essential Physical Requirements: Ability to transfer/maneuver objects weighing at least 50 lbs, frequent pushing, moving, lifting of patients, positioning, bathing, ambulating, reaching overhead, stair climbing, fine motor manipulation. Essential Mental Abilities: Assess patient condition, formulate plan of care, select interventions, evaluate responses, teach patients, prioritize duties, learn new skills and techniques. Essential Sensory Requirements: Visual assessment for dressing changes, phlebotomy, IV regulation; hearing for auscultation, alarms, communication. Exposure to Hazards: Noise, bloodborne pathogens, needle puncture wounds, potential personal safety threats, traffic hazards, weather extremes. Hours of Work: Variable; Mon‑Fri, weekends and holidays as needed. Population Served: Adolescents, adults, geriatrics, and pediatrics. Transportation: Valid North Carolina driver’s license and an operational vehicle required. Benefits and Compensation Hourly Rate: $30.50 – $33.00, based on market rates and individual qualifications.
