Schedule: 7 Days a Week

Morning Shift: 8:30 AM – 12:30 PM

Evening Shift: 5:30 PM – 9:30 PM

Total: 8 Hours per day (Can be filled by one or multiple caregivers)

Job Description

We are seeking compassionate, reliable, and detail-oriented caregivers to assist a client transitioning home from a skilled nursing facility. This role is vital in ensuring the client remains safe, comfortable, and supported during their recovery.

The ideal candidate will be comfortable following a structured care plan and maintaining accurate clinical logs.

Key Responsibilities

Personal Care: Providing essential assistance with showering, toileting, and dressing.

Mobility Support: Assisting the client in and out of bed and ensuring safety during transfers.

Clinical Tracking: Recording daily vitals in a Weekly Clinical Log.

Detailed Care: Following a specific Morning and Evening Daily Care Checklist to ensure all needs (nutritional, hygiene, and comfort) are met.

Requirements

Proven experience in home care or a similar healthcare setting.

Strong communication skills and an empathetic approach to senior care.

Ability to maintain clear and accurate daily documentation.

Reliable transportation to ensure punctuality for scheduled shifts.

Georgia CNA.PCA, or HHA certification, preferred.
