Job Title: Home Care RN Location: Little Rock, AR 72211 Coverage Area: Pulaski, Saline, Jefferson, Faulkner counties Work Schedule: Monday–Friday, 8:00 AM–5:00 PM; occasional weekends and on-call as needed Pay: $54.00-$56.00 per hour Scheduled pay: Weekly Benefits: Health, Dental, Vision, Life Insurance, and Short term disability after 90 days Job Type: Full time Ready to apply or have questions? Contact- Ashley Kruger Call/Text - View phone number on click.appcast.io Email- View email address on click.appcast.io Home Care RN Responsibilities Pulaski, Saline, Jefferson, Faulkner counties Perform comprehensive assessments of high-risk patients/members Collaborate with primary care providers to implement individualized care plans Identify, address, and continuously reassess appropriate and cost-effective levels of care Coordinate non-medical support (ex: housing, transportation) to improve treatment-plan compliance Coordinate care transitions with physicians, social workers, discharge planners, and other partners Engage specialty resources as needed to support best outcomes Maintain detailed documentation of clinical, functional, and outcome measures Promote health education, illness prevention, and patient advocacy while protecting privacy/confidentiality Home Care RN Required Skills & Qualifications Active Arkansas RN license and/or Compact RN required Willing to travel within coverage area: Pulaski, Saline, Jefferson, Faulkner counties 1 year Home Health experience OASIS, and Homecare Homebase (HCHB) experience required Ready to apply or have questions?
