Case Manager/ Support Services

Case Manager Posted Thursday, June 18, 2026 at 4:00 AM Support Services/ Multi-Locations Rate of Pay $65,000/Annually FLSA Status: Exempt Position Summary Oversee and coordinate person-centered services for children and adults across multiple locations, primarily in Hartford County. Primary Job Functions Monitor service delivery, ensure quality outcomes, provide staff supervision and training, and collaborate with leadership to maintain effective, compliant program operations. Role Overview Provide case management services to the individuals served by Richcroft, Inc. as per policies, procedures, and regulations. Monitor the quality of services via visits and communication with each person/ family receiving support services. Collaborate with external organizations on the development of the annual Person-Centered Plan for each person receiving support (PCP). Monitor progress of the outcomes from the PCP monthly Progress notes for all individuals on case load to be completed as outlined in the Individual Plan and submitted to director. Maintain all program files following established guidelines. Provide timely and effective responses to calls. Provide support, training, and day-to-day guidance to staff and individuals of Richcroft, Inc. Keep the Director well informed of any unusual events involving staff, individuals, or the community in a timely manner. Communicate with external organizations as needed. Maintain all required licenses and certifications. Formal supervision with direct support staff. Bi-weekly supervision with Director. Bi-weekly payroll review and approval. Weekly monitoring of LTSS for EVV accuracy. Make corrections and edits to LTSS EVV punches as needed. Review and processing of staff reimbursement requests. Participate in staff interviews, hiring, and on-boarding process. Monitor and update staffing needs. Maintenance of Outlook Calendar and working schedule to complete duties as required. Other duties as assigned. Qualifications Associate degree required; degree in Human Services, Social Work, Psychology, Education, or a related field preferred. Minimum of two years of experience working with individuals with developmental disabilities. Knowledge of person-centered planning and service coordination principles. Strong organizational, communication, and interpersonal skills. Ability to supervise, train, and support direct care staff. Valid driver's license with an acceptable driving record. Ability to work a flexible schedule, including evenings and weekends as needed. Experience An Associates of Arts degree. College education is preferred in the areas of Human Services. Working Conditions Some evenings and weekends are required. Some lifting up to 50 lbs.
