Join our dedicated team at Green Palms Health & Rehab of Wintersville as a Licensed Practical Nurse (LPN) on our Night Shift team!

As an LPN, you will be responsible for providing high-quality, patient-centered care to our residents, working collaboratively with our interdisciplinary team to deliver exceptional care.

Responsibilities:

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring

• Develop and implement individualized care plans, ensuring patient needs and preferences are respected and addressed

• Collaborate with the healthcare team to deliver comprehensive care, including reporting changes in patient condition and participating in care conferences

• Perform routine tasks, such as vital signs, admissions, and discharges, while maintaining accurate and detailed records

• Maintain confidentiality and adhere to HIPAA guidelines
