Marshfield Clinic Eau Claire is seeking a Nurse Practitioner to join thriving practice in Eau Claire, Wisconsin. This position will be working at off-site at Dialysis centers in Eau Claire and Rice Lake. Hours: Monday - Thursday ( 4, 8-hour days)
The Nephrology Nurse Practitioner provides advanced nursing care and management to patients with acute and chronic kidney diseases, including those on dialysis and post-transplant. Working in collaboration with nephrologists and the interdisciplinary team, the NP performs assessments, orders and interprets diagnostic tests, develops and implements treatment plans, and provides education to patients and their families. Conduct comprehensive health assessments including histories and physical exams, focusing on renal health. Manage patients with chronic kidney disease (CKD), end-stage renal disease (ESRD), acute kidney injury (AKI), and hypertension. Order, perform, and interpret diagnostic tests such as lab work, urinalysis, and imaging studies. Prescribe medications, manage dialysis care, and monitor patient responses to treatment. Collaborate closely with nephrologists, dialysis teams, dietitians, social workers, and other healthcare professionals. Educate patients and families on disease processes, medications, treatment options, lifestyle modifications, and transplant considerations. Monitor and adjust care plans for dialysis patients in both inpatient and outpatient settings. Participate in quality improvement initiatives and help develop evidence-based care protocols. Document all patient interactions and clinical decisions in the electronic health record.
Master's or Doctorate degree from an accredited Nurse Practitioner program. If APRN was licensed prior to August 1, 1995, master's degree in nursing is preferred. Nephrology or internal medicine experience preferred. Current licensure as a Registered Nurse and Nurse Practitioner in each state of practice. Current Drug Enforcement Administration (DEA) permit to prescribe controlled substances in each state of practice. State Controlled Substance License (if required by State) National board certification as an Adult Gerontology Acute Care Nurse Practitioner (AGACNP-BC) or ACNPC-AG), Acute Care Adult (ACNP-BC); Additional certifications require approval from a market executive leader: Family (FNP-BC) or (FNPC), Adult-Gerontology Primary Care (AGPCNP-BC) or (A-GNP) Obtains and subsequently maintains required specific competencies and certifications as outlined on privilege form. Privileging documents and related credentialing requirements will supersede this job description in defining minimum qualifications, criteria, and scope of practice, if variation between documents exists.
