Job Summary and Qualifications We are looking for a Kidney Transplant Coordinator who is a registered nurse that performs a variety of clinical services and patient care activities in collaboration with a multidisciplinary healthcare team to ensure the smooth flow of the transplant evaluation and post‑transplant care process. Essential Job Functions Assessments: Perform appropriate assessments including psychological, physical, environmental and functional needs. Education: Provide patient and family education specific to the disease process; serve as a resource for patient, family, staff, hospital, and community. Communication: Communicate with all members of the transplant team, including hospital staff and referring physicians. Data Collection: Participate in collection/entry of data in the patient medical record as required by regulatory agencies. Documentation: Document assessments, interventions and follow‑up accurately and timely. Co‑ordination of care: Work through the phases of transplantation with the transplant team. Qualifications Graduate of an accredited registered nursing program; BSN preferred. Minimum 1 year experience. Complete initial and ongoing training and competencies defined by the service line, facility, and department/unit. A valid Texas RN license; BCLS.
