Outpatient Physical Therapist

This is a 13-week contract position.

Shift: 5x8.0-801361

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with patient needs, goals, and physician direction. The therapist is responsible for the delegation and direction of all discipline-specific therapy services to assigned patients, ensuring the design of functional treatment programs that facilitate an effective return to community activities. This role includes educating patients, families, students, coworkers, and self, while performing other professional duties associated with patient care and the development of discipline-specific therapy services.

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals aimed at progressing patients towards discharge.
• Document medical records in a clear, concise, accurate, and timely manner, ensuring compliance with state, federal, and intermediary guidelines.
• Participate in student programs, providing a positive learning environment through appropriate guidance and evaluations.
• Create individualized professional development plans, actively pursuing growth through continued education and professional involvement.
• Adhere to the discipline-specific State Practice Act and maintain professional licensure in good standing.
• Demonstrate awareness of organizational performance improvement initiatives and contribute to feedback mechanisms for quality improvement.

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• CPR certification.
• Active and applicable license for the state where work will be performed.
