Details Home health setting covering the North Region, including Chelsea, East Boston, Revere, and Winthrop 13-week assignment with a schedule of 8:00 AM to 4:30 PM Weekend rotation required once every 6 weeks Ability to travel to patient cases as assigned within the coverage area Uses Epic for charting and documentation Uses UKG/Kronos for time keeping Requirements Minimum 1 year of Certified Home Health Care experience specifically within a certified home health agency Minimum 1 year of Case Management experience Hospice, private care, or standalone Case Manager experience does not meet the qualifying experience requirement License required if applicable Active MA license required at time of consideration; pending or conditional licenses not accepted AHA certifications only Flu vaccine required with no religious or medical exemptions allowed Additional Information Responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and families Performs admission processes and case management duties in accordance with organizational policies and standards of practice Requires thorough and timely electronic documentation and interdisciplinary communication Start date is contingent on credentialing clearance; delays may result in a rescheduled start date Local candidates accepted if permanent residence is within 60 miles of Boston, MA, at a reduced rate American Traveler Job ID P-732931.
