Behavioral Health Assistant
Help individuals build healthier lives by connecting behavioral health, primary care, and community support in a collaborative, person-centered role. Schedule: Mon-Fri, 8am - 5pm
The Behavioral Health Assistant is an integral member of our interdisciplinary care team, supporting whole-person health for individuals enrolled in our Primary Care Health Home program. Working alongside behavioral health and medical providers, you'll help individuals navigate services, strengthen self-management skills, overcome barriers to care, and stay engaged in their health and wellness goals. This role combines behavioral health support, care coordination, health education, and community resource navigation to improve health outcomes through compassionate, trauma-informed, and person-centered care. While this position does not provide clinical diagnosis or therapy, it plays a meaningful role in helping individuals access the right services at the right time.

Key Responsibilities
- Conduct behavioral health, substance use, health risk, and social needs screenings using established tools and protocols.
- Support integrated care by coordinating communication between behavioral health, medical providers, and community resources.
- Provide health education, wellness coaching, and reinforcement of healthy coping and self-management strategies.
- Assist individuals with navigating referrals, accessing services, and overcoming barriers to care.
- Conduct follow-up outreach to encourage engagement, reinforce care plan goals, and support continuity of care.
- Monitor and support individuals with ongoing behavioral health needs under the supervision of licensed behavioral health professionals.
- Promote a safe environment by responding appropriately to medical or behavioral emergencies in accordance with organizational training and protocols.
- Maintain accurate documentation, reliable attendance, and perform other duties as assigned.
Requirements, Skills, Knowledge and Expertise
- High School/GED required
- Associate's/Bachelor's degree in a human service-related field required. Must qualify for position based on overall CSS requirements in at least one of the options listed:
- Qualified addiction professional
- Bachelor's degree in a human services field
- Any four-year degree or combination of higher education and qualifying experience
- Associate of Applied Science in Behavioral Health Support degree as designated by the Department of Mental Health
- All higher education degrees must be from a college or university included in the U.S. Department of Education's database of accredited schools at
- Qualifying experience must include delivery of service to individuals with mental illness, substance use disorders, or developmental disabilities.
Licensure/Certification:
- Must obtain and maintain current CPR certification within 60 days of hire.
- Must obtain and maintain certification in an approved crisis intervention methodology (CPI, MANDT, or CALM) within 60 days of hire.
Other Qualifications:
- Valid, unrestricted driver's license and agency-established minimum automobile insurance coverage required.
- Demonstrated ability to engage individuals from diverse backgrounds in a respectful, person-centered manner.
- Strong communication, organizational, and problem-solving skills.
