RN Home Care Assessment Nurse BrightStar Care of W. Central Las Vegas & Henderson

Pay: $45.00 per hour
Schedule: Flexible PRN or Part-Time

BrightStar Care is seeking an experienced Registered Nurse (RN) to provide in-home client assessments, reassessments, and supervisory visits throughout the Las Vegas Valley.

This position is ideal for an RN who enjoys working independently, building relationships with clients, and maintaining a flexible schedule without being tied to a hospital or facility.

Rather than providing extended bedside care, you'll focus on evaluating clients, developing and updating care plans, supporting caregivers through supervisory visits, and helping ensure the highest standards of clinical care.

What to Expect

As an RN Home Care Assessment Nurse, you will travel to clients' homes to complete:

• Initial Start of Care assessments (completed within 24 hours of admission)
• Routine reassessments
• Care plan updates
• Supervisory visits for caregivers and skilled nurses

Most assessment and supervisory visits take approximately one hour to two hours , including  electronic documentation.

Hours are scheduled based on client admissions, reassessments, supervisory requirements, and your submitted availability. This position offers flexibility but does not guarantee a set weekly schedule.

Daily travel throughout the Las Vegas Valley is required.

Primary Responsibilities

Clinical Assessments

• Conduct comprehensive in-home client assessments
• Complete Start of Care assessments within 24 hours of admission
• Evaluate changes in client condition
• Develop and update individualized plans of care
• Identify safety concerns and recommend appropriate interventions

Clinical Oversight

• Perform supervisory visits for caregivers and licensed nurses
• Provide clinical guidance and education to field staff
• Ensure compliance with physician orders and care plans
• Maintain BrightStar Care and Joint Commission standards

Documentation & Communication

• Complete accurate electronic documentation following each visit
• Communicate client updates with the clinical office team
• Collaborate with caregivers, nurses, physicians, and client families as needed

Service Area

This position requires travel throughout:

• Las Vegas
• Henderson
• Summerlin
• Green Valley
• Boulder City
• Surrounding communities

Reliable transportation is required.

Qualifications

• Active Nevada Registered Nurse (RN) License
• Minimum two (2) years of nursing experience (home care experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent documentation and communication abilities
• Reliable transportation
• Valid Nevada driver's license and current auto insurance
• Current BLS Certification
• Current TB clearance (2-step TB test or acceptable alternative)
• Ability to lift, push, and pull up to 50 pounds
• Comfortable working in homes where pets may be present

Please Note

This position focuses on in-home assessments, supervisory visits, care plan development, and clinical documentation. It is not a bedside shift nursing, private duty nursing, or case management position.
