PRN Hospice RN

Pioneer Medical Center is a 25-bed critical access hospital, essentially the county hospital for Sweet Grass County, Montana: Population 3700. We are in a beautiful valley along the Yellowstone River, between three mountain ranges, just north of Yellowstone National Park. We serve our neighbors (ranchers, skilled trades people and active retirees) and the many tourists who come here on their way to Yellowstone and Glacier National Parks, or to fish the Yellowstone, hike, bike, hunt or just get away from overcrowded places. Big Timber is a vibrant and welcoming small town, an hour from Bozeman and Billings. Our schools are highly rated, with high community support.

PURPOSE/DISTINGUISHING CHARACTERISTICS:

The PRN Hospice Nurse is responsible for the holistic nursing care of the patient. Maintains excellent relationships with all primary constituencies; patient and families, employees, Interdisciplinary Team (IDG) members, and providers. RN/LPN must provide hospice care within scope of practice and state regulations. There are three available openings for this position.

ESSENTIAL JOB FUNCTIONS

NO. DESCRIPTION

Meets all mandatory organizational and departmental requirements. Maintains competency in all organizational, departmental, and outside agency standards as it relates to the environment, employee, patient safety or job performance. Provides skilled nursing care, patient visits, and assessments according to patient’s plan of care and needs. Assists with picking up on-call shifts and providing patient visits if needed during scheduled call shift. Assists with the admission process as directed by the Hospice Manager. Notifies and/or consults with patient’s attending provider, as appropriate and obtains/implements provider orders. Implements nursing intervention based on nursing assessment of patient and assesses patient outcome. Documents clearly and concisely in a timely manner, nursing care rendered, patient/family observations, teaching plans, consultations, etc. Facilitates hospice transfer to inpatient setting, notifying hospital or skilled nursing facility admissions department/supervisor, and/or emergency room of hospice patient’s identity. May accompany patient/family on transfer to inpatient setting, advocating patient’s desire for palliative care. Answers patient/family questions and concerns relative to hospice care and services, condition changes, and plan of care. Provide comfort and support to patients and families during the final moments of life. Perform all necessary procedures and maintain skillful, compassionate care during the patients transition and for immediate post-mortem procedures. Assist with the patients discharge. Notifies patient’s PCP/Medical Director of patient’s death. Actively participates with the IDG in development of plan of care as appropriate. Assists in educating, supervision, and counseling of patient/family regarding physical care, symptom management, and pain control. Maintains competency in all organizational, departmental, and outside agency safety standards as it relates to the environment, employee, and patient/resident safety. Identifies needs and sets goals for own growth and development. Perform other duties as assigned or needed to meet the needs of the department/organization.

KNOWLEDGE, SKILLS AND ABILITIES

KNOWLEDGE OF:

Common policies and procedures, both organizational and departmental Compliance Program Code of Conduct Regulatory standards (i.e., State Survey, OSHA) HIPAA and confidentiality requirements Patient safety standards Sensitivity of the dynamics of death, dying, and bereavement. Commitment to the hospice philosophy or care. Current Montana driver’s license and mechanism for transportation. Effective verbal, nonverbal, and written communication skills, and leadership qualities. Personal qualities enabling him/her to work effectively with patients, families, staff, and providers. Utilizes proper body mechanics while performing all physical care. Ability to lift fifty (50) pounds and perform personal care on a patient who weighs up to two hundred fifty (250) pounds. Physical and emotional health necessary for the accomplishment of the job. A desire for continued personal and professional growth and development.

MINIMUM QUALIFICATIONS:

Hospice nursing experience or an equivalent combination of education and experience relating to the above tasks, knowledge, skills, and abilities will be considered Professional knowledge of nursing theory and practice acquired through an accredited school of nursing. Current Montana state license to practice as a registered nurse or licensed practical nurse

WORKING CONDITIONS:

Patient care visits in home, nursing home, and hospital settings. Category I: Task that may involve exposure to blood, body fluids and/or tissues.
